Physicians examination hospital workflow

We have described how each physician examination works. Please select the corresponding button.

If this is your first visit

You will need a letter of referral at your first visit to this hospital (however, pregnant and birthing patients are excluded).

1.Complete physician examination application form

Please complete a physician examination application form at the table with writing stationery.

2.To the desk for first time patients

Please present your insurance card and letter of referral. Your basic patient information will be registered into our system (If you are already registered, we will need to verify you).

Patients without a letter of referral will be required to pay JPY 3,300 (taxes included) as fee for treatment of patients’ choice, in addition to physicians examination cost but only at the patient's first visit, per MHLW Notice No 107. *This price of JPY 3,300 (taxes included) has been in effect since April 1, 2016.

3.Calling desk

We will take your physicians appointment for the day and provide you with a hospital appointment slip.

4.Information desk for each specialist / outpatient waiting room

Please wait in the outpatient waiting area of the department until your name is called.

5.Physicians examination, testing, x-rays
 
6.To accounting desk or automated payment machine

Please have prescription for an offsite pharmacy or medicine slips, and have receipt for medicine. If you need to make your next appointment, please take an appointment slip.

*Please discuss the date and time of your appointment with the physician at your examination. We will give you your appointment slip after your bill at accounting is settled.

*You can reduce your waiting time by using the automated payment machine for payment and for making your next appointment.

7.Your medicine

When you have a prescription for an offsite pharmacy:
We will stamp the verification stamp section of your offsite pharmacy prescription if you will let somebody know at the accounting desk or at your medicines desk.

  • Please note that prescriptions with no verification stamp are not valid.
  • In general, our prescriptions are to be used by an offsite pharmacy. Please present them to a pharmacy covered by your insurance carrier within 4 days from the date on which you were given the prescription.
8.On returning home
 

If you will be seen by different medical department for the first time

Please take your ID card and appointment slip, and wait in the waiting area of your testing room.

  • You can arrange a follow-up visit on the machine with your ID card.
  • If your health insurance card has not been verified by us for more than 3 months, we will take your card at the follow-up visit desk.

*Even if you have an appointment, if you come on a different date or you are unable to access the follow-up appointment machine, please come to the follow-up visit desk (for follow-up patients with no appointment)

Notes for when you are here for your appointment

Appointment for patients under the age of 16 (physicians examination)

When there is no guardian or caretaker accompanying the patient, this patient will neither be examined by the physician nor be given a prescription for drugs. When visiting the hospital for an examination, we ask that underage patients are accompanied by his/her/their guardian or caretaker.

Please do not lose your ID card

Please do not lose your ID card. Store it carefully and always bring it with you when you come for a physicians examination. If you lose or damage your card and require your card to be re-issued, the cost will be JPY 200 (taxed included).

How to make an appointment

Please consult with your physician during your appointment about the date and time of your next visit. We will give you an appointment slip for your next visit when you finish paying the bill for your current visit.

On payments

To reduce your waiting time at the desk, you also can use our payment machine.

  • *If the payment machine is not working, please come to the accounting desk.
  • *We cannot re-issue your receipts, please do not lose them.
  • *If you need an itemized receipt of your bills (items you paid), please ask for this at the accounting desk.

You can use credit cards to pay for your physicians examination cost and such as of January 2009.

If you need documents such as letter of diagnosis and other formal letters, they can be obtained from the 1F general information desk at the hospitalization and discharge & documentation desk.

Other

This hospital is a non-smoking zone. Please respect our policy.

When you must use a mobile telephone or other telecommunication device, they may affect precision instruments such as medical devices, so when you must use them at our hospital, please respect that they can only be used in designated areas and they are forbidden in other areas.

*PHS used by our hospital staff is PHS for the purpose of medical care only.

  1. Please insert your ID card into the ID card / hospital card opening.
  2. Please confirm the details on the screen and press enter.
  3. Please take your appointment slip.
  4. This completes your appointment.

How to operate the payment machine

  1. Please insert your ID card into the ID card / hospital card opening. Or, please place the outpatient card barcode at a position which allows the red beams of the payment machine to read the barcode.
  2. Please confirm the amount of payment on the screen.
  3. Please select your method of payment (cash / credit card).
    (1) If you are payment by cash
    1. Please insert your bills and coins.
    2. Please confirm the details on the screen and press enter.
    3. Please take your change.
    4. Please take the receipt and your credit card.
    5. This completes your payment.
    (2) When you are paying by credit card
    1. Please insert your credit card and enter your PIN number.
    2. Confirm the details on the screen and press enter.
    3. Please take the receipt and your credit card.
    4. This completes your payment.
